Why You May Want to Submit a Data Access Correction Request

Under the General Data Protection Regulation (GDPR), you have the right to request correction or updates to personal data we hold about you. This is known as the right to rectification.

A data correction request allows you to:

  • Correct inaccurate or incomplete personal data
  • Update outdated information
  • Ensure your records are accurate and reflect current circumstances
  • Resolve any errors in how we’ve recorded your information
  • Maintain data integrity for your own protection

You may want to submit a correction request if you’ve noticed errors in your personal information, changed your name or address, or need to clarify details we’ve recorded.

Note: If you would also like to request access to your personal data, you can make that request in the “Additional Requests” section of the form. 


How to Complete and Submit Your Request

Step 1: Download or Access the Form Locate the Data Correction Request Form below and download or open it in your browser.

Step 2: Provide Your Information Complete all required fields, including:

  • Your full name
  • Email address and/or mailing address
  • Any identifying information that helps us locate your records (e.g., date of participation in a study, company name, or employee ID)

Step 3: Specify the Data to Be Corrected Clearly describe which personal data you believe is inaccurate or incomplete and explain what the correct information should be. Include specific examples and dates if possible.

Step 4: Additional Requests (Optional) Use the “Additional Requests” section of the form if you would like to request access to your personal data or make any other related requests. 

Step 5: Verify Your Identity Include a copy of a government-issued ID or other proof of identity to confirm you are the individual whose data you’re requesting to update. This protects your privacy and security.

Step 6: Submit Your Form 


What to Expect

We will acknowledge receipt of your request and review the data you’ve identified within 30 days. We’ll contact you if we need additional information or clarification. Once verified, we will correct the data and inform you of the changes made. We will also notify any third parties to whom we’ve shared the inaccurate data, where appropriate.
